Goal Setting Idea Checklist

 Goal 1  Healthy Eating:  I will eat more fruits, vegetables, whole grains
and lean meats.  I will eat less sweets and fatty foods.
 Goal 2  I will try to make my food serving sizes smaller.
 Goal 3  I will try to lose weight.  I will exercise daily and be more active.
 Goal 4  I will take my medicine and check my blood sugar as directed
by my doctor.
 Goal 5  I will check my bare feet daily.
 Goal 6  I will brush my teeth at least 2 times per day and floss once daily.
 Goal 7  I will see my doctor at least every 3 months.
 Goal 8  I will stop smoking.
