Daily Self-Care Checklist
| 1. | Check your blood sugar before meals and at bedtime. |
| 2. | Exercise daily. |
| 3. | Follow a meal plan. |
| 4. | Take care of your heart. Check your blood pressure and know what your cholesterol levels are. |
| 5. | Take your medicine daily as directed by your doctor. |
| 6. | Take care of your feet. Use a mirror to check the bottoms of feet and in between toes. |
| 7. | Take care of your teeth. Brush at least 2 times a day and floss daily. |
| 8. | Manage stress and depression. |

| 11. | Create goals for yourself. |
| 12. | Make doctors appointments and get the tests you need on a regular basis. |
